ADLs
Bathing / Showering Goals:
- Patient will improve ability to safely and efficiently bathe self, including washing, rinsing, and drying self with SUP or touching assistance with use of long-handled sponge and with use of shower chair.
Personal Hygiene & Grooming
Toileting Goals:
- Patient will safely perform toileting tasks using grab bars with independence with ability to right self to achieve/maintain balance.
- Patient will safely perform toileting tasks using grab bars with independence to return to PLOF.
- Patient will perform toileting tasks using standard commode with independence in order to return to PLOF.
- Patient will perform toileting tasks using standard commode in order to facilitate independence and good hygiene when home.
- Patient will improve ability to perform toilet transfers with safety awareness in order to return to PLOF.
Dressing Goals:
- Patient will safely perform LB dressing with independence with use of AE in order to return to PLOF.
- Patient will safely perform LB dressing with independence without use of AE in order to return to PLOF.
- Patient will safety donn/ff footwear with independence with use of AE in order to return to PLOF.
- Patient will improve ability to safely and efficiently put on and take off socks and shoes or other footwear with setup or clean-up assistance with use of adaptive equipment.
Feeding Goals:
- Patient will demonstrate ability to scoop and transport food from surface to mouth using AE to independently complete self-feeding.
- Patient will independently complete self-feeding using least restrictive assistive device.
Transfer Goals:
- Patient will safely perform functional transfers ______ with least restrictive assistive device to maximize independence to participate in ADLs.
- Patient will safely perform functional transfers ____ with least restrictive assistive device to participate in ADLs and return to PLOF.
- Patient will complete ADL transfers with independence and least restrictive assistive device to safely return to PLOF.
- Patient will complete ADL transfers/mobility with independence and least restrictive assistive device in order to safely return to PLOF.
Functional Mobility – WC Goals:
- Patient will increase ability to safely propel self in wheelchair 30 feet with SUP on level surfaces in order to facilitate increased participation with functional daily activities.
Standing Tolerance Goals:
- Patient will increase standing tolerance to 3-5 minutes to facilitate independence in ADLs and functional mobility.
- Patient will increase standing tolerance to 3-5 minutes to participate in ADLs and return to PLOF.
- Patient will demonstrate 5 mins static standing tolerance while maintaining _______ to promote functional independence in BADLs.
- Patient will stand supported for 3-5 mins to return to PLOF and facilitate participation in ADLs.
- Patient will demonstrate 5 mins static standing tolerance while maintaining weight-bearing (R/L UE/LE) orders.
Activity Tolerance Goals:
- Patient will increase activity tolerance for functional activities of choice to 15 mins in order to increase participation with ADL tasks.

MMT:
- Patient will increase (L or R) shoulder flexion strength to (#)/5 in order to increase participation in UB self-care tasks.
